Patient Information
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, and major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Pay Early and Save!
If you pay for your procedures in advance, we will deduct 5% of your portion of the payment. This option is not available when using CareCredit.
In-Office Financing
Upon approval by Dr. Kelly, patients may qualify for interest-free financing over a period of three months. Monthly payments of an equal amount must be made on time during the three-month window. If a longer period of time than three months is needed, we offer financing through outside companies.
Dental Insurance
As a service to our patients, we will file your insurance claims on your behalf. While we will file claims with all insurance companies, we are contracted providers with two insurance companies: Delta Dental and Health Resources, Inc.
